Can I schedule both my follow-up visit and annual physical at the same time?: Yes, when you have your biometric testing done at Oz Health you will be asked to schedule a follow-up appointment to discuss the results, you may also schedule your physical at that time. Physicals must be done by December 31, 2015 to be eligible for the wellness payment.

6. What do I need to do to schedule an appointment?: Simply call the clinic at (262) 536-4344, please keep in mind that this is a 12 hour fasting blood draw. If you have specific health requirements that make it difficult to fast please discuss this with the Oz Health staff.

7. If I need a blood draw do I need to have it done at Oz Health?: Yes, the clinic has early morning hours and later hours on Wednesdays. We have also planned to have additional staff available to meet demand. Unless, you have had/or will have it done in the 6 months that precede your HRA. If you have had the biometric screening done before, simply request the Primary Care Provider form from the HR Director.

8. Do I need to have my annual physical before April 1, 2015?: No, you may have it done at any point in 2015 to be eligible. You still will have to provide an EOB or signed certification from your Doctor to be eligible.

10. Will the County have access to my health information?: No, each login is for the individual employee and spouses. The County will not have access to any information other than which employees have participated so that payments can be made.

11. What will the HRA information be used for?: The PopSpring portal will use the HRA information for predictive modeling of our insured employees. For example, if the HRA data demonstrates that our members have a higher than average incidence of blood glucose as a group (not by individual employee), we will work with PopSpring to develop wellness programming aimed at controlling pre-diabetic conditions.

12. How secure is the PopSpring website?: The website is fully secure and PopSpring is fully HIPAA compliant, your health information is treated exactly the same is if it was being shared with any health care provider.

13. Can I request a copy of my biometric results done at the clinic for my primary care physician?: Yes, simply call the clinic at (262) 536-4344, you will have to physically pick up a copy of the results or sign a release.
