Service Providers

Directory of Services

Aging and Disability Resource Center of Ozaukee County maintains a Directory of Services for our clients use to help them connect with local services to meet their needs. This directory is updated annually and we are always interested in including new providers.

To be considered for inclusion an organization must:

  • Have been providing services in Ozaukee County for at least 1 year
  • Complete a Request for Information form and return to the ADRC

Completing the Request for Information form does not in any way guarantee inclusion in the Directory of Services. All requests are reviewed by the ADRC of Ozaukee County Director for inclusion.

Approved organizations will be added to the directory during the next update. Organizations failing to update their information may be removed.
