Vision Insurance (Must work a minimum of 18.75 hours to be eligible)

Ozaukee County's vision insurance is administered through MetLife.

• The employee is eligible for Vision Insurance on the first day of the month following the completion of two calendar months of employment.
• The pre-taxed Vision deductions are taken a month before the premium is due.
• MetLife does not issue ID cards for vision coverage. You would need to use our group number or your Social Security number at your provider's office.

                   Single - Plus Spouse: $15.42

                   Single-Plus Child(ren):  $16.15
